SWICA und Medbase setzen sich für eine integrierte medizinische Versorgung ein und engagieren sich gemeinsam für eine optimale Betreuung bei chronischen Erkrankungen. Seit 2018 setzt Medbase in ihren Medical Centern das strukturierte Betreuungskonzept für die Behandlung von Diabetes Typ 2 ein. Das Winterthurer Institut für Gesundheitsökonomie der ZHAW untersucht den Erfolg dieses Konzepts in einer Begleitevaluation. Die aktuelle Analyse zeigt: Die Behandlungsqualität konnte nachhaltig verbessert werden – bei markant tieferen Kosten.

Strukturierte Behandlungsprogramme, sogenannte Disease-Management-Programme, tragen wesentlich zu einer qualitativ hochwertigen und kosteneffizienten Versorgung von chronisch kranken Menschen bei. 2018 haben SWICA und Medbase gemeinsam ein strukturiertes Betreuungskonzept für Patientinnen und Patienten mit Diabetes Typ 2 etabliert. Das Winterthurer Institut für Gesundheitsökonomie (WIG) untersucht, wie sich dieses Programm auf die Qualität der Behandlung und die Gesundheitskosten auswirkt. Dazu werden die Abrechnungsdaten von SWICA von entsprechend betreuten Patientinnen und Patienten den Daten einer Vergleichsgruppe gegenübergestellt. Zusätzlich werden anonymisierte Daten aus Konsultationen der Medbase-Praxen ausgewertet.

Bessere Qualität und 11,8 Prozent tiefere Kosten   
Die Analyse der Abrechnungsdaten von 2023 zeigt, dass die Behandlung immer konsequenter gemäss den medizinischen Leitlinien umgesetzt wird. Diese umfassen die regelmässige Messung des Blutzuckerwerts, ein jährliches Lipidprofil, die Erfassung des Zustands der Nieren und alle zwei Jahre eine Augenuntersuchung. Seit der Einführung des strukturierten Betreuungskonzepts im Jahr 2018 ist der Anteil der Personen, deren Behandlung vollständig den Leitlinien entsprach, gegenüber der Kontrollgruppe angestiegen. Dieser positive Effekt zeigte sich unmittelbar nach der Einführung des Betreuungskonzepts und blieb danach stabil oder nahm leicht zu. Dies unterstreicht den nachhaltigen Effekt auf die Versorgungsqualität der Diabetikerinnen und Diabetiker.

Die Gesundheitskosten stiegen bei Patientinnen und Patienten im strukturierten Betreuungskonzept deutlich moderater als in der Kontrollgruppe. Ab dem dritten Jahr nach Einführung des Programms lagen die jährlichen relativen Einsparungen bei etwa 1 500 bis 2 000 Franken pro Patientin oder Patient. Im Jahr 2023 betrug der Spareffekt 1 873 Franken pro Person. Damit liegen die durchschnittlichen Behandlungskosten 11,8 Prozent tiefer als in der Vergleichsgruppe.

Erfolgsfaktor Zusammenarbeit   
Die enge Zusammenarbeit zwischen Medbase, SWICA und dem WIG ist ein wesentlicher Bestandteil der Umsetzung des strukturierten Diabetes-Programms. Die Ergebnisse unterstreichen das grosse Potenzial strukturierter Behandlungskonzepte und betonen die Bedeutung ihrer konsequenten Umsetzung sowie der fortlaufenden datenbasierten Weiterentwicklung.

| Wie funktioniert das integrierte Behandlungskonzept bei Diabetes? Das integrierte Behandlungskonzept für Diabetespatientinnen und -patienten bietet einen individuellen Behandlungsplan, basierend auf anerkannten, evidenzbasierten medizinischen Leitlinien. Der Plan beinhaltet die Vereinbarung von Therapiezielen zwischen den betreuenden Gesundheitsfachpersonen und den Patientinnen und Patienten. Davon werden Massnahmen abgeleitet, die unter anderem die Anpassung der medikamentösen Behandlung, Beratungstermine, regelmässige Kontrolluntersuchungen oder physiotherapeutische Trainingseinheiten beinhalten können. Die Behandlung wird dokumentiert, regelmässig evaluiert und gegebenenfalls angepasst. Aufgrund der Betreuung durch ein interprofessionelles Team erhalten die Patientinnen und Patienten ausführlichere Beratungen und eine engere Begleitung als bei ausschliesslich ärztlichen Konsultationen. |

#### Über uns

Die Handelskammer und Arbeitgebervereinigung Winterthur HAW ist 1992 aus der Fusion der Kaufmännischen Gesellschaft Handelskammer Winterthur und dem Arbeitgeberverband Winterthur entstanden. Ihre Wurzeln gehen bis ins Jahr 1801 zurück. Die HAW prägte die wirtschaftliche Entwicklung der Stadt Winterthur und trägt bis heute dazu bei, dass Winterthur ein Wirtschaftsstandort mit gut ausgebauter Infrastruktur ist.
