**Probleme rund um die Medikamenteneinnahme verursachen zahlreiche Spitalaufenthalte und sind immer wieder für Todesfälle verantwortlich. Im Rahmen des integrierten Versorgungsnetzwerks Trio+ setzen sich das Kantonsspital Winterthur, Medbase und SWICA für mehr Medikamentensicherheit für Winterthurer Patientinnen und Patienten ein. **

Unerwünschte Nebenwirkungen, Wechselwirkungen oder falsche Medikamente: Medikationsfehler kommen auch in unserem fortschrittlichen Gesundheitssystem noch viel zu häufig vor. Gemäss der Stiftung Patientenschutz Schweiz sind über 20 000 Spitalaufenthalte pro Jahr auf medikamentenbedingte Probleme zurückzuführen. Jede zehnte Person in einem Schweizer Spital wird durch Medikationsfehler oder unerwünschte Wirkungen geschädigt.

**Trio+ bietet die Voraussetzungen für effiziente Zusammenarbeit  
**Das Kantonsspital Winterthur (KSW), Medbase und SWICA setzen sich im Rahmen von Trio+ für mehr Sicherheit bei der Arzneimittelabgabe und -einnahme ein. Seit 2022 spannen die drei Partner aus Winterthur zusammen, um die integrierte Versorgung in der Region Winterthur voranzutreiben. Für den Einsatz für mehr Sicherheit im Umgang mit Medikamenten eignet sich diese Kooperation ideal. Denn gemäss Christian Ambrosch, Leiter Medbase Winterthur Neuwiesen und Facharzt für Allgemeine Innere Medizin, gehen besonders beim Spitaleintritt und -austritt beziehungsweise beim Übergang von der ambulanten zur stationären Behandlung und umgekehrt häufig Informationen verloren. «Es passieren Fehler oder es gibt Missverständnisse. Darum arbeiten wir daran, die Dokumentation und die Kommunikation zu verbessern», erklärt Ambrosch. «Ein zentraler Punkt ist auch, dass die Patientinnen und Patienten miteinbezogen und zur aktiven Teilnahme an ihrer Behandlung motiviert werden. Dafür sind Unterstützung und Schulung wichtig.»

**Mehr Sicherheit mit dem eMediplan  
**Hier kommt der eMediplan ins Spiel. Der elektronische Plan listet alle Medikamente auf, die eine Patientin oder ein Patient aktuell einnimmt. Er beinhaltet unter anderem die Namen und Fotos der Medikamente, Details zur Dosierung und Einnahme, den Grund für die Verschreibung und den Namen der Ärztin oder des Arztes, der das Medikament verordnet hat. Alle Informationen auf dem Plan sind im integrierten QR-Code enthalten. Die Fachpersonen können diesen Code einlesen und wenn nötig anpassen. Patientinnen und Patienten können den Code mit ihrer App scannen und haben den Plan dann auf ihrem Smartphone zur Verfügung. Fragen dazu können sie direkt mit den Fachpersonen besprechen.

Im April 2024 hat Trio+ ein Pilotprojekt gestartet, bei dem der eMediplan bei den verschiedenen Partnern etabliert wird und allfällige Lücken in der Versorgungskette geschlossen werden. Gemäss Béatrice Tissot, Unternehmensentwicklung & Strategisches Management beim KSW, ist das ein Schritt in die richtige Richtung. «Schweizweit gibt es schon seit Jahren Bemühungen, den eMediplan einzuführen, aber es mangelt an einer breitflächigen Umsetzung. Mit dem konsequenten Einsatz legen wir eine wichtige und notwendige Grundlage, um die Medikamentensicherheit zu erhöhen», sagt Tissot.

**TRIO+: integrierte Versorgung in der Region Winterthur  
**SWICA sieht in der integrierten Versorgung einen zentralen Ansatzpunkt zur Verbesserung des Gesundheitssystems. Darum setzt sich die Gesundheitsorganisation zusammen mit dem KSW und Medbase mit Trio+ für die integrierte Versorgung in der Region Winterthur ein. #### Über uns

Die Handelskammer und Arbeitgebervereinigung Winterthur HAW ist 1992 aus der Fusion der Kaufmännischen Gesellschaft Handelskammer Winterthur und dem Arbeitgeberverband Winterthur entstanden. Ihre Wurzeln gehen bis ins Jahr 1801 zurück. Die HAW prägte die wirtschaftliche Entwicklung der Stadt Winterthur und trägt bis heute dazu bei, dass Winterthur ein Wirtschaftsstandort mit gut ausgebauter Infrastruktur ist.
